Job summary

MediLodge of Zeeland is seeking a Social Services Worker to provide psychosocial support to residents and their families in a long-term care setting.

You will conduct resident assessments, develop care plans, and coordinate with external services to support discharge and family coping. A SOcial Work background with state licensing compliance is required or preferred, with ongoing education and team collaboration emphasized.

Qualifications

• Bachelor-level social work degree preferred
• Licensing as required by the state of employment
• Minimum one year of experience in a long-term care environment

Responsibilities

• Provides direct psychosocial intervention.
• Performs resident assessments at admission, upon condition change and annually.
• Creates, reviews and updates care plans and progress notes.
• Coordinates resident visits with outside services (dental, optical, etc).
• Attends and documents resident council meetings.
• Assists residents’ families in coping with skilled nursing placement, illness and disabilities.
• Works with patient, family, and team to plan discharge.
• Conducts in-service programs to educate staff on psychosocial issues and patient rights.
