Job summary

MediLodge of St. Clair seeks a Social Worker to provide psychosocial support to residents and families. You will assess residents, develop and update care plans, coordinate external services, and assist families with coping during skilled nursing placement.

You will educate staff on psychosocial issues, participate in discharge planning, and supervise Social Services Assistants, contributing to patient-centered care and staff development.

Qualifications

• Education: BSSW, MSSW preferred not needed.
• Experience: One year of experience in a long-term care environment.

Responsibilities

• Provides direct psychosocial intervention.
• Performs resident assessments at admission, upon condition change and/or annually.
• Creates, reviews and updates care plan and progress notes.
• Coordinates resident visits with outside services, dental, optical, etc.
• Attends and documents resident council meetings.
• Assists resident’s families in coping with skilled nursing placement, physical illness and disabilities of the resident, and the grieving process.
• Works with the patient, family, and other team members to plan discharge.
• Conducts in-service programs to educate staff regarding psychosocial issues and patient rights.
• Supervises and guides Social Services Assistants.
• Performs other tasks as assigned.
