Job summary

Cedar Hills Center for Nursing and Rehabilitation is seeking a compassionate Social Worker to support residents and families throughout their healthcare journey. You will advocate for residents, coordinate services, assist with discharge planning, and promote rights, dignity, and quality of life.

Working with an interdisciplinary team, you will develop individualized care plans, provide emotional support, connect families with community resources, and maintain confidential documentation.

Qualifications

• Bachelor's degree in Social Work or related field.
• Healthcare or long-term care experience preferred.
• Strong communication, organization, and interpersonal skills.
• Excellent documentation and case management abilities.
• Ability to maintain confidentiality and exercise professional judgment.
• Compassionate, resident-centered approach.
• Advocacy for residents and families.
• Ability to work independently within an interdisciplinary team.

Responsibilities

• Complete psychosocial assessments and care plans.
• Assess psychosocial and emotional needs; develop interventions.
• Participate in interdisciplinary care plan meetings.
• Provide counseling and guidance to residents and families.
• Assist with discharge planning and transitions.
• Coordinate family meetings and resources.
• Maintain communication with residents, families, physicians, and care team.
• Collaborate with nursing, therapy, activities, dietary and others.
