Chelsea Jewish Lifecare is seeking a Social Worker to conduct comprehensive psychosocial assessments, develop individualized care plans, and provide counseling to residents and families. You will collaborate with an interdisciplinary team to coordinate services and support smooth transitions.

The role requires a MSW/BSW and current MA LSW/LCSW/LICSW license, with long-term care experience preferred and English fluency for communication.

Qualifikationen

• Bachelor or Master of Social Work (BSW or MSW).
• Current, unencumbered MA LSW, LCSW, or LICSW license.
• Prior experience in a long-term care setting strongly preferred.
• Ability to communicate fluently in English - both verbally and written.

Aufgaben

• Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs.
• Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
• Connect residents and families with community resources and post-discharge support services to promote successful care transitions.
