Stratford Manor Rehabilitation & Care Center in New Jersey is seeking a compassionate Social Worker to join our Skilled Nursing Facility team. You will support residents and families with emotional support, discharge planning, care coordination, and advocacy to ensure the highest quality of care and life.

The ideal candidate holds a BSW/MSW, current licensure, and experience in healthcare settings; strong communication and interpersonal skills; and a resident-centered approach.

Qualifications

• Bachelor’s or Master’s degree in Social Work (BSW/MSW).
• Current state licensure (as required).
• Experience in a skilled nursing or healthcare setting preferred.
• Strong communication, organizational, and interpersonal skills.
• Compassionate and resident-centered approach.

Responsibilities

• Conduct resident assessments and develop individualized care plans.
• Provide counseling and emotional support to residents and families.
• Coordinate discharge planning and post-acute services.
• Collaborate with nursing, therapy, and interdisciplinary teams.
• Ensure compliance with state, federal, and facility regulations.
• Assist with admissions, transitions, and resident advocacy.
