MB Healthcare's Stratford Manor Rehabilitation & Care Center is seeking a Social Worker to support residents and families with emotional support, discharge planning, care coordination, and advocacy to enhance quality of life.

The ideal candidate holds a BSW/MSW, current state licensure, and experience in skilled nursing or healthcare. You will collaborate with nursing and therapy teams to deliver person-centered care.

Qualifications

• Bachelor’s or MSW required.
• Current state licensure as required.
