Responsibilities

• Assist residents and families with transition into the facility.
• Complete a comprehensive Psychosocial Assessment identifying social, emotional, and psychological needs.
• Identify medical social needs and provide appropriate services.
• Collect and maintain resident social information in admission files.
• Maintain lists of community resources for Social Services and clinical care.
• Plan, organize and implement Social Service programs.
• Evaluate effectiveness of Social Service programs with the team and other staff.
• Assess residents for discharge and coordinate discharges.
• Advocate for residents' best interests and total care in Social Services and clinical care.
• Conduct resident interviews and ensure Care Area Assessments meet regulatory requirements.
• Perform other related job duties as required.
