Job summary

Heritage Grove Estates in Topeka, Kansas seeks a Social Services Assistant to support residents’ social, emotional, and psychosocial needs in a long-term care setting. You will work under the Administrator and Director of Social Services.

Responsibilities include implementing the social services program, coordinating resources, discharge planning, and collaborating with the interdisciplinary team while educating residents about rights and available services.

Qualifications

• Bachelor’s degree in Social Work or related human services field.
• Minimum one year supervised social work experience in healthcare.
• Strong communication and documentation skills.
• Ability to work with residents, families, staff and community agencies.

Responsibilities

• Implement the facility’s social services program.
• Assess resident social service needs and coordinate resources.
• Assist residents and families with discharge planning and referrals.
• Coordinate care with the interdisciplinary team (IDT).
• Participate in care planning and MDS social service components.
• Provide education on resident rights and services.
• Assist with admissions, transfers, and discharges.
• Maintain compliance with state and federal regulations.
