Job summary

The position involves developing social histories and care plans, documenting progress, and participating in interdisciplinary care planning, with a full-time, on-site schedule. Training and ongoing education are provided.

Qualifications

• High school diploma is required.
• SSD certificate or willingness to obtain.
• Ability to be physically present onsite at the community.
• Good listening skills.

Responsibilities

• Develop a social history, social assessment and care plan for each new resident.
• Document progress notes related to each resident's plan of care.
• Participate in interdisciplinary Care Plan meetings and review care plans.
• Assist in community education regarding aging, residents' rights and facility services.
