Job summary

Beaver Dam Healthcare Center is seeking a Director of Social Services to oversee social work services for residents, evaluate needs, and coordinate with the interdisciplinary team. This leadership role includes guiding case management and ensuring quality resident support.

The ideal candidate holds a Bachelor's in social work, at least one year in health care, and strong communication and computer skills. Experience in post-acute care or outpatient rehab is preferred.

Qualifications

• Bachelor’s Degree in social work (SW) or social welfare from an accredited academic institution.
• At least 1 year experience in health care setting.
• Excellent written and verbal communication skills.
• Computer proficiency with Microsoft products.

Responsibilities

• Provide social services (social work) to residents.
• Evaluate residents and identify areas of concern.
• Coordinate services with the interdisciplinary team.
• Oversee case management services for residents and review resident status.
