Job summary

Signature HealthCARE in Kentucky seeks a Social Services Director to address residents' social, emotional, and psychological needs and assist in discharge planning.

You will document compliance with state and federal policies, participate in care planning, and serve as a resource for residents and families. The role requires a Bachelor’s/Master’s in Social Work or Psychology and 3+ years in social services, ideally in healthcare.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• 3+ years of experience in Social Services, preferably in healthcare.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation to comply with policies.
• Participate in resident care planning.
• Participate in discharge planning and prepare residents for transition.
• Serve as a general source of information for residents and families.
