Job summary

Socket.dev is seeking a licensed Social Worker to lead the social services department in a long-term care facility. You will advocate for residents, plan services, and coordinate with the Administrator to meet federal, state, and local standards.

You will develop, implement, and monitor social services, ensure proper documentation, and participate in discharge planning and family support. Collaboration with staff and community partners is essential.

Qualifications

• Must be a licensed Social Worker in good standing with the state.
• Must interact positively with residents, families, regulatory agencies and coworkers.
• Must maintain attendance and be able to work long or irregular hours, often under pressure.

Responsibilities

• Develop, implement, and direct all social services functions.
• Ensure an on-going Quality Assurance program and develop plans to correct deficiencies.
• Consult with and coordinate social services activities with other departments and implement changes as required.
• Ensure that forms, reports, and progress notes are properly completed and descriptive.
• Participate in community planning, discharge assessments, and discharge plans.
• Coordinate social services with residents, families, staff, and community agencies.
• Provide information to families about support agencies and financial assistance programs.
• Maintain resident confidentiality and treat residents with kindness, dignity, and respect.
• Monitor residents' emotional and social needs and ensure staff are knowledgeable about these needs.
• Perform other duties as assigned.
