Job summary

Villa Healthcare is seeking a Social Services Designee/PRSC to provide person-centered social, emotional, and psychological support to skilled nursing residents. The role focuses on advancing residents’ growth and potential within a respectful, dignified environment.

The Designee will manage discharge planning, coordinate with families and staff, and ensure compliance with relevant regulations. Experience with geriatric care and knowledge of Medicare/Medicaid coverage are preferred.

Qualifications

• Must have high school diploma or equivalent.
• Experience with geriatric residents preferred.

Responsibilities

• Keeps informed about community resources and communicates with staff about admissions and discharges.
• Discusses options for advanced directives and guardianship with residents or representatives.
• Documents discharge planning and coordinates with family to assess needs.
• Identifies resident psychosocial, cultural, ethnic, and spiritual needs.
• Assists in end-of-life, palliative and hospice discussions.
• Supports case manager with insurance coordination and discharge arrangements.
• Completes required assessments and documentation in a timely manner.
