Job summary

Villa Healthcare seeks a Social Services Designee/PRSC at Westland Villa Center to provide person-centered social, emotional, and psychological support to skilled nursing residents. You will coordinate with families, staff, and case managers to ensure smooth admissions, discharges, and care planning.

Responsibilities include advocating for residents, discussing advance directives, and ensuring regulatory compliance while delivering compassionate, respectful service in a skilled nursing setting.

Qualifications

• Must read, write, and communicate in English.
• Experience with geriatric residents preferred.
• Knowledge of Medicare, Medicaid, HMO and private insurance coverage.
• Familiarity with accrediting standards and IDPH/state/federal regulations.

Responsibilities

• Keep informed about community resources for residents.
• Communicate with staff about admissions and discharges to units and facilities.
• Discuss advanced directives, financial power of attorney, guardianship, and surrogate decisions with residents or representatives.
• Identify and report abuse per policy and relevant laws.
• Document discharge planning and communicate with family as discharge approaches.
• Address residents' psychosocial, cultural, ethnic, and spiritual needs.
• Discuss end-of-life care (POLST), palliative care, and hospice with residents or families.
• Assist Case Manager with insurance coordination and discharge needs.
• Ensure home care needs are met before discharge, with planning completed 24 hours prior.
