Villa Center in Michigan is seeking a Social Services Designee/PRSC to support skilled nursing residents with social, emotional, and psychological needs, ensuring person-centered care and continuous growth. The role requires coordinating with residents, families, and staff, and guiding decisions on directives, guardianship, and discharge planning.

Qualified candidates will have a high school diploma; experience with geriatric populations is preferred.

Qualifications

• Must have a high school diploma or equivalent.
• Experience with geriatric residents preferred.
• Knowledge of Medicare/Medicaid and accrediting standards.

Responsibilities

• Maintains knowledge of community resources and coordinates with residents and families.
• Communicates admissions/discharges with units, facilities, hospitals, and home care.
• Discusses advanced directives, financial power of attorney, and guardianship with residents or representatives.
• Supports abuse prevention and reports deviations per policy and law.
• Documents discharge planning and coordinates arrangements 24 hours prior to discharge.
• Addresses psychosocial, cultural, ethnic and spiritual needs.
• Assists with end-of-life care discussions and POLST.
• Collaborates with Case Manager on insurance considerations.
• Supports home care arrangements and timely assessments.
