Job summary

Qualifications

• Strong communication and organizational skills.
• Experience in healthcare or social services preferred.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Assist in developing and implementing policies and procedures for the department.
• Maintain regular Social Service progress notes and resident information.
• Coordinate social services with other departments and community resources, including discharge planning.
