Responsibilities

• Coordinates and directs Social Services in accordance with federal, state and local regulations, guidelines and as directed by the Administrator.
• Responds to resident behavioral and psychiatric issues by completing psychosocial assessments and making referrals to mental health providers.
• Coordinates discharge plans with residents and families and assists with advance directives.
• Resolves and tracks grievances and coordinates family council meetings.
