Job summary

HealthRIGHT 360’s North County Serenity House seeks a Case Manager to coordinate intake, assessments, and benefits, working with a multidisciplinary team to support recovery. This in-person role requires reliable transportation and a passion for client-centered care.

You will assist with Medi-Cal applications, document progress, and ensure HIPAA and agency standards. The program serves women in residential settings with opportunities for growth and impact.

Qualifications

• Option I: High school diploma or equivalent.
• Substance Use Disorder Counselor registration from CA agencies (CCAPP, CAADE, CADTP).
• Option II: Bachelor’s degree in Social Work, Psychology, or related field.
• Case management experience.
• Experience in human services and relapse prevention.
• First Aid and CPR certification or ability to obtain within 30 days.
• Valid CA Driver’s License and insured transportation.

Responsibilities

• Assist client intake by completing case management assessment and entering financial and benefit information.
• Support client in applying for Medi-Cal or transferring Medi-Cal County when appropriate.
• Assess client’s case management needs and complete all releases of information.
• Connect client to benefits, mental health, physical health, employment, housing, and aftercare.
• Coordinate communication with probation, DCFS, and other services; schedule appointments and arrange transportation.
• Complete client UA tests and document results.
• Write progress notes within 72 hours and ensure documentation compliance.

Education

High school diploma or equivalent; SUD Counselor registration (CA: CCAPP, CAADE, CADTP)

Bachelor’s degree in Social Work, Psychology, or related field

Case management experience

Experience in human services and recovery

First Aid and CPR certification or ability to obtain within 30 days

Valid California Driver’s License and insured transportation
