Job summary

Community Partners in Action seeks a Case Manager at Hartford Transitional House (HTH) in Hartford, CT. This role focuses on assessments, service planning, and coordinating residential programming for residents reentry and accountability.

The ideal candidate holds a Bachelor's degree with at least one year of case management experience, strong communication skills, and preferably bilingual English/Spanish. A valid driver's license is required to support referrals and transportation.

Qualifications

• Bachelor's degree plus one year experience in assessments and case management preferred.
• Equivalents will be considered in lieu of education.
• Knowledge of criminal justice and court systems helpful.

Responsibilities

• Conduct resident intake, admission and assessment (LSI-R) using motivational strategies.
• Develop the Residential Individual Service Plan based on assessment outputs.
• Provide recommendations for coordinating and sequencing program services.
• Coordinate and follow up on resident referrals to community-based services.
• Monitor resident behaviors and implement program behavior management.
• Prepare accurate progress and discharge reports.
• Input resident data into the Contractor Data Collection System (CDCS).
• Supervise urine tests as required.
• Administer PREA assessments within required timeframes.
