Job summary

Community Partners in Action seeks a Case Manager for Hartford Transitional House in Hartford, CT. The role centers on intake, assessments, and developing a Residential Individual Service Plan to support residents’ progress and transition goals.

You will coordinate services, document progress, and collaborate with community partners. A Bachelor’s degree and strong communication are required; bilingual English/Spanish preferred.

Qualifications

  • Bachelor's degree plus one year experience in conducting assessments and case management duties.
  • Knowledge of criminal justice and court systems; ability to interact with people of diverse backgrounds.
  • Strong written and oral communication skills; experience in assessments and case management preferred.
  • Must possess strong computer skills; bi-lingual English/Spanish preferred.

Responsibilities

  • Conduct resident intake, admission and assessment (LSI-R) using motivational enhancement strategies.
  • Develop the Residential Individual Service Plan based on assessment outputs.
  • Provide recommendations for the coordination and sequencing of program services for residents.
  • Coordinate and follow up on resident referrals to community-based services, as necessary and appropriate.
  • Prepare accurate and timely progress and discharge reports.
  • Input resident data into the Contractor Data Collection System (CDCS) and maintain data as directed by the Program Manager.
  • Utilize CPA’s case management system (Securmanage) as directed by the Program Manager.
