Job summary

Diversicare Healthcare Services, LLC in Alabama seeks a compassionate Certified Nurse Assistant to support residents with daily activities, mobility, and personal care.

You will accompany residents to medical appointments, document visits, and ensure safety during transport, while upholding our Service Standards and core values.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair or vehicle.
• Possess a valid state driver’s license.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the guidelines at each visit: sign in resident, present documents, obtain prescriptions and appointment cards as needed.
• Report to RCM after returning to facility and be prepared to answer questions about the visit.
• Performs other duties as required and assigned.
