Job summary

Diversicare Healthcare Services, LLC is seeking a compassionate Certified Nurse Assistant to support residents in our care centers. You will assist with transfers, accompany residents to medical visits, and help maintain safety and comfort during transport.

Responsibilities include documenting visits, obtaining prescriptions as needed, and communicating with the RCM. A valid CNA credential and clean driving record are required for this full-time role.

Qualifications

• CNA certification and credentialing required.
• Ability to assist residents with transfers and mobility.
• Valid state driver’s license and clean driving record.
• Criminal background generally clear and compliant with state rules.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits and stay with them until the visit is complete.
• Observe guidelines at each visit and present documents to office staff as needed.
• Obtain any new prescriptions and ensure proper doctor’s order documentation.
• Report back to the facility RC M and answer questions about visits.
• Performs other duties as required and assigned.
