Job summary

Star of Hope is seeking a Care Coordinator to guide residents through their life cycle, from enrollment to exit, providing ministry support, goal development, and one-on-one case management. The role emphasizes accountability, documentation, and collaboration across departments within a faith-based mission.

Responsibilities include weekly meetings with residents, service-plan updates, and connecting individuals to counseling, education, and community resources.

Qualifications

• BA/BS in social services or related field; 1 year of related work experience or equivalent
• At least 1 year experience working with homeless or formerly homeless individuals or families preferred
• Experience with individuals with substance use and/or mental illness is helpful
• Valid Texas driver’s license with acceptable driving record and current liability insurance
• Must be detail oriented, well organized; strong written and oral communication; public speaking ability

Responsibilities

• Manage resident life cycle from enrollment through progression or exit with goal planning and accountability
• Assess resident needs and complete enrollments, exits, class assignments, services, and updates in CaseWorthy
• Meet with residents weekly to develop and update service plans and monitor progress
• Connect residents to counseling, education, workforce development, medical care and benefits assistance
• Maintain accurate resident documentation and communicate updates to the manager
• Support program safety and adherence to rules, policies, and conflict resolution
• Participate in care coordination meetings and collaboration with staff and volunteers
• Provide guidance to new staff and support ministry activities from a Biblical perspective
