Star of Hope is seeking a Care Coordinator to guide residents through their life cycle, from enrollment to exit, providing ministry support, goal development, and one-on-one case management. The role emphasizes accountability, documentation, and collaboration across departments within a faith-based mission.
Responsibilities include weekly meetings with residents, service-plan updates, and connecting individuals to counseling, education, and community resources.

The Care Coordinator manages and walks alongside residents during their life cycle within the Star of Hope Resident Journey. This includes ministry support, resident assessment, goal development, short- and long-term planning, one‑on‑one case management, accountability, conflict resolution, grievance support, discipline, termination processes when appropriate, and accurate documentation. The Care Coordinator ensures residents are connected to appropriate programming, services, classes, and resources that support their transition from homelessness toward healing, restoration, transformation, purpose, self‑sufficiency, and sustainable stability. The position also supports facility order, program compliance, and collaboration across departments.
Education, Skills & Experience: B.A. or B.S. in social services or other related field, with one year of related work experience; or an equivalent combination of education and experience. At least one year experience working with homeless or formerly homeless families/individuals preferred. Experience working with individuals with substance use and/or mental illness is helpful. Valid Texas driver’s license with acceptable driving record and current liability insurance.
Must be detail oriented, well organized and able to update and maintain accurate resident case files in a timely manner. Must have strong communication skills, both written and oral, and conflict resolution skills. Ability to speak in public and counsel individuals one‑on‑one. Proven problem‑solving ability and able to work in a team. Must be able to learn and retain information quickly in a fast‑paced environment. Must have good judgement in dealing with confidential Star of Hope matters and resident information.
Be able to work respectfully, professionally and in a Christ‑like manner with everyone, able to maintain required resident/staff boundaries and possess a willingness to serve the Lord in a ministry that deals with homeless men, women and children.
General Office Supplies, Computer Equipment, Copier/Fax/Printer, Cleaning Supplies, Microsoft Word, Excel, PowerPoint, Outlook, and case management databases such as CaseWorthy or designated systems. May drive a Star of Hope 12 or 15 passenger van when assigned and properly authorized.
Work schedule may vary and be subject to change to meet the business needs of the mission, including early, evening, and/or weekend shifts with rotating holiday schedules. Local travel between mission facilities and occasional travel within the city is required. Personal Protective Equipment (PPE) is required as needed for completion of tasks. Exposure to infectious diseases, poor hygiene, drug usage, verbal and physical abuse and other emergency situations related to the homeless population is normal. Business casual dress in accordance with the Star of Hope dress code.
Constant contact with residents. Constant face‑to‑face discussions with individuals. Frequent contact with immediate peers, immediate manager and volunteers. Frequent use of telephone conversation, electronic mail, written letters and memos. Occasional contact with peers and managers in other departments and other agencies. Occasional public speaking.
Light work with lifting of up to 20 pounds. Constant sitting, use of fingers, handling, talking, hearing, close vision at 20 inches or less, and repetitive motion. Frequent, reaching, tasting/smelling, distance vision, field of vision, and depth perception. Occasional balancing, stooping, kneeling, crouching, standing, walking, handling, feeling, pushing, pulling, and color vision.
