Job summary

Blaire House of Worcester is seeking a Social Worker to join our 75-bed skilled nursing center dedicated to dementia and Alzheimer’s care. The role focuses on psychosocial assessment, patient advocacy, and discharge planning within a compassionate, family-owned setting.

Responsibilities include developing social histories, providing counseling as needed, maintaining progress notes, and coordinating with families and community resources to ensure smooth transitions for residents.

Qualifications

• B.A. or M.A. degree in social work or related field.
• Licensed Social Worker at the LSW level or higher.
• At least one year direct social work experience in a long term care facility.

Responsibilities

• Develop a comprehensive social history.
• Providing social intervention, such as counseling, as patient advocate, as liaison with the community, or regarding financial or emotional issues.
• Maintaining social service progress notes on patient medical records which reflect changes in psycho-social or physical status, evaluation of family interactions, identification of new problem areas, and review of social plans, goals and progress.
• Develop initial discharge plan and document in clinical record, revising until discharge with available community services.
