Job summary

Pleasant Acres Nursing & Rehabilitation Center is seeking a Social Worker to join our caring care team. You will help evaluate and ensure the well-being of residents, collaborating with a multidisciplinary staff to maintain individualized care plans.

In this role you will support planning, conduct assessments, coordinate admissions and discharges, and document in PointClickCare. We value compassionate, patient-centered service and a commitment to dignity for all residents.

Qualifications

• Two years of social work experience in a long-term care facility.
• BSW or Bachelor's degree in a related field.
• Experience using PointClickCare or other health information management system; proficiency using Microsoft Word, Excel and Outlook.

Responsibilities

• Support the Social Services Director with department planning to meet residents’ medical, emotional and social needs.
• Develop and maintain rapport with patients/residents, families, interdisciplinary team members, service providers and facility staff to support social services programs.
• Participate in admission and discharge planning, conduct assessments, care planning processes and coordinate room changes.
• Enter assessments and notes into PointClickCare and department logs; maintain survey-ready records.
• Investigate residents’ complaints and assist in resolution.
• Assist in purchasing clothing, toiletries, or other items when funds allow, or may be assigned to make purchases for residents.
• Other duties as assigned.
