Job summary

Hallettsville Nursing and Rehabilitation Center seeks a licensed social worker to oversee admissions, assess resident needs, and coordinate referrals to community resources. Qualifications

• Bachelor’s or Master’s in Social Work.
• Must be a Licensed Social Worker in the state of Texas.
• Experience in a skilled nursing facility preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Oversee admissions process and identify resident needs.
• Make referrals to community resources and coordinate social services.
• Ensure residents' mental and social health is supported; counsel residents and families.
• Complete psychosocial assessments and contribute to care planning and MDS when needed.
• Participate in care plan meetings, QA reporting, and department updates.
• Assist with resident/family concern management and routine investigations when required.
