Responsabilités

• Establishes course of action by exploring options with resident and families.
• Assist residents in achieving highest practicable level of self-care, independence and well-being.
• Provide social services to attain or maintain highest practicable physical, mental and psychosocial wellbeing.
• Evaluate social and family information to assess social services needs and develop care plans.
• Provide emotional support to residents and families dealing with disability, health concerns, or loss.
• Assist residents with health care decisions and discharge planning.
• Coordinate outpatient services and arrange on-site ancillary services.
