Job summary

North Shore Healthcare now hiring a Social Services professional to support residents in a post-acute care setting. This role evaluates resident needs, coordinates services with families and staff, and oversees case management to ensure quality outcomes.

Strong communication and documentation are essential. Preferred candidates hold a social work license, a bachelor’s degree in social work or welfare, and prior post-acute care experience.

• Computer proficiency with Microsoft products.

Responsibilities

• Assess residents and evaluate needs for social services.
• Coordinate social services with residents, families and the interdisciplinary team.
• Oversee case management and collaborate with community resources as needed.
• Maintain timely and accurate documentation in the resident chart.
• Provide training and participate in clinical conferences as required.
