Job summary

Monarch Healthcare Management seeks a Social Services Designee to support residents through admission and discharge, market services, and build strong referral-source relationships. You will clarify legal documents for residents and families and help develop individualized plans of care within an interdisciplinary team.

The role focuses on psychosocial assessment, care planning participation, and ensuring alignment with residents’ rights and facility policies across Monarch's Minnesota

Qualifications

• Must recognize and understand the psychosocial and emotional needs of residents.
• Must have strong skills in communication, organization, management, and leadership
• Must be able to perform essential functions of the position with or without reasonable accommodation.
• Experience in healthcare, skilled nursing facility or assisted living community

Responsibilities

• Admission and Discharge Coordination: manages referrals, pre-admission screenings, paperwork, and discharge plans.
• Resident and Family Support: provides emotional support, guidance, and information to residents and families.
• Communication and Liaison: acts as a link between residents, families, staff, and community agencies.
• Care Planning Participation: completes social assessments, PHq-9, BIMs, MDS, and care plan documentation.
• Professional Conduct and Relationship Management: maintains professional relationships and adheres to policies.
• Facility Compliance and Education: promotes QAPI initiatives and regulatory compliance
