Job summary

Qualifications

• Must possess, as a minimum, a high school diploma or its equivalent; Associate or Bachelor's Degree preferred but not required.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs.
• Meet with administration, medical and nursing staff, and other related departments in planning social services.
• Assist in developing and coordinating department policies and procedures.
• Review department policies and procedures annually and participate in changes.
• Assist in identifying and meeting the medically related social and emotional needs of residents.
• Coordinate social service activities with other departments and community agencies.
• Maintain regular Social Service progress notes indicating response to treatment plans and adjustments.

Education
