Qualifications

• High school diploma or equivalent required; associate or bachelor's preferred.
• Healthcare experience, especially long-term care, preferred.
• Knowledge of OBRA and HIPAA regulations.
• Strong communication, organizational, and documentation skills.
• Empathetic, patient, and supportive attitude.

Responsibilities

• Assist residents with psychosocial needs, including adjustment to facility life and emotional support.
• Conduct assessments and collaborate on care planning for residents with the interdisciplinary team.
• Maintain accurate documentation in line with state and federal guidelines.
• Advocate for residents' rights and mediate concerns or grievances as needed.
• Coordinate social services interventions and refer residents to external agencies (e.g., mental health, hospice, housing).
• Assist with discharge planning, including home services, equipment, and family communication.
• Participate in care conferences and collaborate with nursing, therapy, dietary, and medical staff.
• Help plan and implement social programs to support residents’ mental and emotional well-being.
• Maintain confidentiality and promote resident dignity and respect.
