Responsibilities

• Conduct resident interviews to gather social, emotional, and financial needs.
• Assist in discharge planning with healthcare providers, families and resources.
• Document interviews, calls, and care conferences daily.
• Coordinate with nurses, therapists and social workers for care planning.
• Lead discharge planning and coordinate with outside resources for safe transition.
