Job summary

Greenwood Healthcare Center, a member of the CommuniCare Family, is seeking a Social Services Designee to support residents’ psychosocial well-being in a long-term care setting. This full-time role involves advocating for residents and coordinating with care teams to meet individual needs.

The ideal candidate will have a high school diploma or GED, prior social services experience in long-term care, and a Social Services Designee certificate.

Qualifications

• High school graduate or GED equivalent.
• Previous social services experience in a long term care environment.
• Social Services Designee Certificate required.

Responsibilities

• Develop and maintain a good working rapport with direct caregivers to ensure participation in programs/services for residents’ psychosocial well-being.
• Report grievances and complaints, and provide oral/written reports to the Director of Social Services.
• Serve as the resident’s advocate in collaboration with direct care staff to meet residents’ needs, including appropriate dress and appearance.
• Be aware of changes in a resident’s condition and report immediately to the RN Charge Nurse.
• Provide information to help solve residents’ problems and better meet their needs.
• Improve the quality of services and activities, increasing opportunities and choices for residents.
• Work with activity department to involve family members and guests in activities when possible.
• Maintain confidentiality of all resident/family information.
