Job summary

Edmond Health Care Center is seeking a registered nurse or licensed practical nurse to provide skilled nursing care under physician direction. You will oversee CNAs, coordinate with the IDT, and ensure comprehensive resident assessments and care plans.

The role includes medication administration, admission/discharge support, and active participation in quality committees and continuous education. The facility emphasizes compassionate, trauma-informed care and adherence to infection control

Qualifications

• Must possess a nursing degree from an accredited college or university or be a graduate of an approved LPN/RN program.

Responsibilities

• Provide nursing care to residents under the medical direction of the attending physician.
• Oversee CNAs and licensed nurses as directed by the Director of Nursing.
• Coordinate with the interdisciplinary team to assess, plan, implement and evaluate resident care plans.
• Complete MDS assessments as requested and prepare physician orders for signature.
• Administer medications per orders and document any issues or adverse effects.
• Participate in quality committees (QAPI, infection control, safety, risk management).
• Assist with discharge planning and ensure new admissions have physician orders for immediate care.
• Maintain documentation in nursing notes and electronic medical records.
• Collaborate with hospice for end-of-life care and promote falls prevention.
