The Resident Care Nurse Coordinator is responsible for supporting the day-to-day clinical and operational functions of the Resident Care department. This role provides hands-on leadership and coordination related to resident care services, medication management, staffing and scheduling support, caregiver oversight, resident documentation, training, and regulatory compliance to ensure exceptional resident experiences and quality outcomes. The Resident Care Nurse Coordinator serves as a key operational and clinical support leader by promoting resident-centered care, hospitality, safety, dignity and continuity of care for residents.

ESSENTIAL FUNCTIONS:

Responsibilities may include, but are not limited to the following:

• Coordinates staffing schedules to support quality resident care, operational continuity, attendance management, and labor efficiency while minimizing overtime and agency utilization
• Provides day-to-day leadership, direction, coaching, accountability and workflow oversight to resident care associates.
• Develops positive relationships with residents and families through proactive communication, responsiveness, empathy, and professionalism.
• Assists in addressing resident and family concerns and escalates clinical or operational concerns appropriately.
• Assist with resident assessments, incident follow-up, physician communication, care coordination, and implementation of individualized service plans as directed by the Resident Care Director.
• Supports survey readiness and compliance with Texas Health and Human Services long-term care regulations, infection prevention standards, medication management protocols, and company policies.
• Leads with a hospitality mindset by creating a calm, welcoming, respectful, and service-focused environment for residents, families, and associates.
• Oversees safe medication administration practices, medication cart organization, narcotic documentation processes, medication pass efficiency, and compliance with medication management standards.
• Enters, reviews, verifies, and maintains resident medication orders within the Electronic Medication Administration Record (eMAR) system to ensure accuracy, completeness, and compliance with physician orders, medication management standards, and community policies.
• Performs daily audits of missed Activities of Daily Living (ADL) documentation and task completion reports, identifies trends or concerns, follows up with associates as needed, and communicates unresolved issues to the Resident Care Director (RCD.
• Maintains organization, cleanliness, inventory accuracy, and regulatory readiness of resident care supply rooms, medication rooms, and related storage areas to support efficient operations, infection control standards, and resident care needs.
• Receives, reviews, and reconciles monthly medication cycle deliveries for accuracy against physician orders, resident medication profiles, and pharmacy documentation; promptly reports discrepancies, missing medications, packaging concerns, or required corrections to the Resident Care Director and pharmacy provider.
• Assists with new resident admissions by obtaining resident photographs for the eMAR system, collecting baseline vital signs, completing initial skin observations/assessments per community protocol, and ensuring accurate documentation and communication to clinical leadership.
• Assists residents with activities of daily living (ADLs), personal routines, and supportive services in a manner that promotes dignity, independence, comfort, safety, and quality of life while maintaining a clean, organized, and welcoming living environment.
• Creates, updates, audits, and maintains resident ADL care profiles and service plans to accurately reflect residents’ current functional status, care needs, preferences, safety requirements, and individualized support needs.
• Participates actively in the onboarding, orientation, training, mentoring, and ongoing development of caregivers and resident care associates, reinforcing community standards, dementia care best practices, infection control procedures, customer service expectations, and resident-centered care principles.
• May supervise Caregivers & CNAs in the delivery of planned resident care.
• Assists the Resident in maintaining a safe and clean environment by surveying the Resident’s room for safety and removing all soiled linens from rooms. Bed linens, towels, and washcloths are to be changed as necessary.
• Performs routine checks on residents.
• Maintains that all resident call lights are promptly answered, followed by appropriate responses to requests.
• Exhibits a caring and respectful attitude toward residents and the residents’ property
• Assist with activities and encourage resident engagement and participation
• Attend bi-weekly in-service meetings and mandatory meetings
• Adheres to all community and company policies
• Maintains confidentiality of all pertinent resident care information (upholds the confidentiality of residents and coworkers, HIPAA)
• Must demonstrate professionalism in appearance, communication, attendance, accountability, teamwork, and adherence to community policies, procedures, and hospitality standards.
• Performs other duties as assigned by the Resident Care Director, LVN/LPN or the General Manager

QUALIFICATIONS AND REQUIREMENTS:

• Graduate from high school or GED required; vocational nursing education, associate degree, or healthcare-related education preferred.
• Current Licensed Vocational Nurse (LVN) license required.
• A minimum of two (2) years of experience in assisted living, memory care, long-term care, skilled nursing, or senior living environments is required.
• Previous experience in staffing coordination, scheduling, medication management, resident care coordination, or supervisory responsibilities preferred.
• Experience working with residents living with Alzheimer’s disease, dementia, cognitive impairment, or behavioral health challenges strongly preferred.
• Have the professionalism to establish and maintain a good rapport with residents and family members. Ability to handle emotionally charged situations professionally.
• Strong leadership, organizational, critical thinking, and problem-solving skills with the ability to prioritize responsibilities in a fast-paced environment.
• Ability to communicate effectively and professionally with residents, families, physicians, vendors, visitors, and interdisciplinary team members.
• Demonstrates emotional intelligence, patience, professionalism, and compassion when working with residents, families, and team members in emotionally sensitive situations.
• Strong customer service and hospitality mindset with the ability to create positive resident and family experiences.
• Basic computer proficiency required, including experience with Microsoft Office, scheduling systems, and email communication.
• Current CPR and First Aid certification required or ability to obtain within designated timeframe.
