The Outlook at Windhaven, part of Forefront Living, is seeking a Nursing Care Coordinator in Plano, TX. This role focuses on leading the Assisted Living team, coordinating resident care, and ensuring personalized, compassionate service for residents and families.

You will collaborate with the Director of Assisted Living and an interdisciplinary team to maintain high standards of clinical care while mentoring staff and maintaining documentation in the EMR.

Qualifications

Responsibilities

  • Serve as liaison between residents, families, and healthcare providers and document in EMR.
  • Monitor meal service, resident flow and day-to-day unit operations.

**Final compensation within the listed pay range will be based on relevant experience, skills, and qualifications, and will align with our established pay scale.

  • Support the Director of Assisted Living with daily department operations, resident care oversight, and follow-up on priority clinical and staffing needs.
  • Complete/Assist with resident level of care assessments, annual reviews, change-in-condition follow-up, and service plan updates based on current resident status.
  • Complete the "Daily Start-Up" review process to identify changes in condition, required notifications and accurate documentation in the electronic medical record. Direct needed follow-up.
  • Attend assigned interdisciplinary service plan meetings and communicate updates to the care team as needed.
  • Help maintain resident rosters, acuity tracking, and other department logs used for staffing, quality review, and state readiness.
  • Provide orientation, feedback and support to clinical staff on expectations, documentation, customer service, safety, infection control, and community procedures.
  • Observe care delivery on the floor and provide real-time follow-up related to quality of care, teammate performance, resident interaction, and policy adherence as needed.
  • Assist with performance evaluations, coaching conversations, and disciplinary follow-up in collaboration with the Director of Assisted Living and Human Resources as applicable.
  • Manage clinical staffing through OnShift. Coordinate call-ins, schedule adjustments, shift coverage, and staffing follow-up in accordance with approved staffing plans and departmental guidelines.
  • Verify payroll-related timekeeping and approved time-off scheduling.
  • Assist with interview screening, onboarding, and orientation of new teammates, including review of work rules, call-in expectations, and schedule processes.
  • Monitor meal service, resident flow, and day-to-day unit operations and assist with problem-solving when immediate support is needed.
  • Investigate resident, family, or teammate concerns brought to the department and report findings and recommendations to the Director of Assisted Living.
  • Maintain confidentiality of resident and teammate information and serve as a liaison with other departments, providers, families, and outside partners.
  • Step into nurse or other direct support duties during staffing emergencies or urgent operational needs, within licensure and training.
  • On call for scheduling during the week and every other weekend for both clinical and scheduling needs.
  • Serve as the primary liaison between residents, families, physicians, specialists, and other healthcare providers by coordinating communication, physician orders, referrals, and follow-up care.
  • Receive, process, and communicate physician documentation, orders, referrals, and recommendations, ensuring timely implementation and accurate documentation in the electronic medical record.
  • Perform other duties as assigned to support the clinical, operational, and staffing needs of the community.
Qualifications:
  • Valid Texas LVN license
  • Ability to perform Essential Duties
  • Genuine caring for and interest in the care of mature adults, persons with disabilities and hospice patients
  • Ability to perform work tasks within the Physical Demand Requirements
  • Ability to comply with the Resident Bill of Rights and Employee Responsibilities
