Resident Care Coordinator / LPN Supervisor

(9 AM-5:30 PM Weekdays | Rotational Weekend MOD)

Wage: $30-$31.50/hr

Summary of Position

The Resident Care Coordinator (RCC) plays an integral role in the success of our team, our community, and creating a place our residents are proud to call home. The primary purpose of this position is to collaborate with the Director of Nursing to plan, organize, develop, and direct the overall functions of resident care services in accordance with current federal, state, and local standards, guidelines, and regulations.

Essential Job Functions
Management Functions
  • Assist in developing, maintaining, updating, communicating, and enforcing facility and state specific written policies and procedures that govern the daily functions of the nursing service department, including the safety program.
  • Maintain a current reference of written nursing material that supports the department’s day-to-day needs.
  • Communicate nursing services schedules with other services to ensure continuity of resident care.
  • Assist in developing, implementing, and maintaining a Continuous Quality Improvement and Quality Assurance program.
  • Recommend equipment needs to the Director of Nursing.
  • Participate with the Director of Nursing in preparing and planning the nursing service department’s budget and staffing plan.
Nursing Care Functions
  • Conduct daily nursing functions in accordance with current rules, regulations, and guidelines that govern the long‑term care facility.
  • Ensure all nursing personnel comply with written policies, procedures, and the safety program.
  • Admit, transfer, and discharge residents with proper documentation.
  • Participate in orientation of new residents and family members.
  • Communicate with healthcare providers in a timely and appropriate manner.
  • Utilize nursing personnel qualifications according to the resident’s care plan and provider orders.
  • Complete daily clinical and environmental rounds and report significant events.
  • Resolve issues with residents and/or family members; report significant issues to the supervisor.
  • Participate in facility surveys (inspections) as requested by management.
  • Follow clinical programs including risk management, infection prevention, quality, and staff development.
Communication/Documentation Functions
  • Complete an initial assessment and develop service plans for assigned residents.
  • Update, review, and transcribe service plans based on nursing needs and new health provider orders.
  • Communicate with healthcare providers to review treatment plans, complete orders, and progress notes.
  • Review resident health records for specific treatments, medication orders, diets, labs, and follow-up requirements.
  • Requisition and arrange diagnostic and therapeutic services as ordered by the provider.
  • Communicate department policies and procedures to personnel, residents, and visitors.
  • Provide written (24‑hour) and oral reports/recommendations regarding resident condition changes and shift activities to nursing management.
  • Fill out and submit Risk Watch incidents involving residents or staff to the Director of Nursing.
  • Complete assessments and all other health record documentation as required to provide care services.
  • Report discrepancies concerning physician orders, diet changes, charting errors, etc. to the Director of Nursing.
  • Maintain clinically accurate documentation in accordance with policy and procedures.
  • Review interdisciplinary team notes to ensure adherence to the service plan.
  • Inform nursing personnel of new admissions and resident care needs; ensure rooms are ready for new admissions.
  • Notify the resident’s attending healthcare practitioner and family of any changes in condition.
  • Meet with support personnel (Housekeeping, Dining Services, Caregivers, etc.) to plan shift services, programs, and activities, and monitor task completion.
  • Coordinate services with interdisciplinary team members to maintain total resident care regimen.
  • Assist in arranging transportation for residents and third‑party services.
Medication Administration Functions
  • Review medication administration records for completeness and accuracy; report concerns to supervisor.
  • Dispose of drugs and narcotics in accordance with established procedures.
Personnel Functions
  • Assist in recruitment and selection of nursing service personnel.
  • Help develop work assignments, schedule duty hours, and supervise nursing staff to ensure compliance with standards.
  • Support staff development; monitor performance and provide coaching, counseling, and discipline as needed.
  • Collaborate with the Director of Nursing and Executive Director to resolve performance issues, including potential termination.
  • Complete timely performance evaluations.
  • Review complaints and grievances, conduct investigations, and work with the Director of Nursing on resolution plans.
  • Participate in planning, conducting, and scheduling in‑service and orientation training that meets state and federal requirements.
Knowledge And Critical Skills
  • Make independent decisions and follow instructions.
  • Deal tactfully with personnel, prospects, residents, families, visitors, government agencies, and the public.
  • Work with ill, disabled, elderly, and emotionally upset individuals.

Education And Experience
  • Minimum of two (2) years of nursing experience.
  • Minimum of one (1) year of leadership or care coordination experience preferred.

Supervisory Responsibility

This position has direct reports and supervisory requirements.

Working Conditions and Physical Demands

Tasks may involve exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
