Job summary

Touchmark on West Prospect in Appleton seeks an experienced Resident Care Manager to coordinate resident care, lead caregiving teams, and ensure person-centered service in our Assisted Living and Memory Care neighborhoods.

The role requires a Bachelor’s degree in health administration, social work, psychology, gerontology, or related field, plus at least one year of supervisory experience and strong leadership, communication, and organizational skills.

• At least one year of successful staff management or supervisory experience.
• Experience with older adults and advancing care needs.
• Experience in assisted living, memory care, or related health care environment preferred.

Responsibilities

• Coordinate and oversee residents’ activities of daily living and individualized care needs.
• Develop, review, and update resident service plans and facilitate care conferences.
• Monitor residents to ensure care needs are met and address changes.
• Serve as a primary contact for residents and families regarding care and concerns.
• Coordinate with nursing, physicians, home health, hospice, and other providers to support continuity of care.
• Assist with resident move-ins and transitions as needs change.
• Ensure caregivers understand and follow resident service plans.
• Provide day-to-day leadership, supervision, coaching, and support to the caregiving team.
• Train and develop caregivers with clear expectations for quality and accountability.
• Participate in recruiting, onboarding, scheduling, evaluating, and retention of caregiving staff.
• Maintain visible presence in neighborhoods, observing resident needs and care delivery.
• Conduct quality assurance activities and ensure compliance with Touchmark standards and regulations.

Resident Care Manager

Touchmark on West Prospect is Appleton’s leading full service senior living community, dedicated to enriching the lives of residents through exceptional care, hospitality, and meaningful relationships.

We are seeking an experienced and compassionate Resident Care Manager to help ensure exceptional, person centered care for residents in our Assisted Living and Memory Care neighborhoods. This position combines resident care coordination, family communication, quality assurance, and leadership of our caregiving team.

The Resident Care Manager is a visible presence in our neighborhoods, developing relationships with residents and families while providing caregivers with the leadership, direction, and support they need to deliver excellent care.

What You’ll Do

• Partner in recruiting, onboarding, scheduling, evaluating, and retaining caregiving team members
• Maintain a visible presence in the neighborhoods, observing both resident needs and the delivery of care
• Conduct quality assurance activities and help ensure compliance with Touchmark standards and applicable regulations

What We’re Looking For

• Bachelor’s degree in health administration, social work, psychology, gerontology, or a related field
• At least one year of successful staff management or supervisory experience
• Experience working with older adults and individuals with advancing care needs
• Previous experience in assisted living, memory care, senior living, health care, social services, or a related environment strongly preferred
• Strong leadership and coaching skills
• Excellent communication and relationship building skills with residents, families, and team members
• Strong organizational skills and ability to coordinate multiple resident and staffing needs
• Understanding of resident care planning and quality assurance
• Ability to work collaboratively with nursing and other health care professionals
• Proficiency with computers, including Microsoft Office
• CPR/AED and First Aid certification as required
