Job summary

Touchmark on West Prospect in Appleton is seeking an experienced Resident Care Manager to lead our Assisted Living and Memory Care teams with person-centered care.

You will coordinate resident care plans, guide caregiving staff, and communicate with residents' families to ensure exceptional service and quality outcomes in a compassionate, compliant environment.

You'll partner with nursing and health services leadership to maintain standards and continuity of care across neighborhoods.

Qualifications

• Bachelor's degree in health administration, social work, psychology, gerontology, or related field.
• At least one year of successful staff management or supervisory experience.
• Experience working with older adults and individuals with advancing care needs.
• Previous experience in assisted living, memory care, senior living, health care, social services, or related environment preferred.
• Strong leadership and coaching skills.
• Excellent communication and relationship building with residents, families, and team members.
• Strong organizational skills and ability to coordinate multiple resident and staffing needs.
• Understanding of resident care planning and quality assurance.
• Ability to work collaboratively with nursing and other health care professionals.
• Proficiency with computers, including Microsoft Office.
• CPR/AED and First Aid certification as required

Responsibilities

• Coordinate and oversee residents' activities of daily living and individualized care needs
• Develop, review, and update resident service plans and facilitate care conferences
• Monitor residents to ensure care needs are being met and changes are addressed
• Serve as a primary point of contact for residents and families regarding care, concerns, and changing needs
• Coordinate with nursing, physicians, home health, hospice, and other providers to support continuity of care
• Assist with resident move ins and transitions as care needs change
• Ensure caregivers understand and follow resident service plans
• Provide day to day leadership, supervision, coaching, and support to the caregiving team
• Train and develop caregivers while establishing clear expectations for quality, service, and accountability
• Partner in recruiting, onboarding, scheduling, evaluating, and retaining caregiving team members
• Maintain a visible presence in the neighborhoods, observing resident needs and delivery of care
• Conduct quality assurance activities and ensure compliance with Touchmark standards and regulations

Touchmark on West Prospect is Appleton's leading full service senior living community, dedicated to enriching the lives of residents through exceptional care, hospitality, and meaningful relationships.

We are seeking an experienced and compassionate Resident Care Manager to help ensure exceptional, person centered care for residents in our Assisted Living and Memory Care neighborhoods. This position combines resident care coordination, family communication, quality assurance, and leadership of our caregiving team.

The Resident Care Manager is a visible presence in our neighborhoods, developing relationships with residents and families while providing caregivers with the leadership, direction, and support they need to deliver excellent care.
