Job summary

Commonwealth Senior Living is seeking an Assistant Resident Care Director to support the Resident Care Director in planning, implementing, and evaluating the Resident Care department. You will provide direct resident care under supervision, role-model the company values, and help lead a team focused on high-quality care and a homelike environment.

The role requires a current LPN license, nursing experience, and CPR/First Aid certifications, with flexibility for weekend/evening hours and on-call

Qualifications

• State-approved LPN license required.
• At least one year of nursing experience, preferably in long-term care.
• Experience in medication administration and geriatrics, psychiatric, or rehabilitation nursing.
• CPR and first aid certification required.
• Demonstrates leadership and sound nursing judgement.
• Willing to train for state assessments and service plans as per licensing.
• Ability to work weekends, evenings, and flexible hours, including on-call.
• Proficient with basic office equipment and computer use.
• Acceptable driving record.

Responsibilities

• Maintain liaison with residents, families, and support departments to plan care.
• Ensure services comply with Federal and State policies governing resident care.
• Deliver direct resident care and supervise direct care staff.
• Report changes in resident condition to the Resident Care Director promptly.
• Assist with scheduling to ensure adequate staffing per policy and budget.
• Complete resident care assessments and individualized care plans.
• Assist with lifts, transfers, and safe mobility for residents.
• Administer and document medications per physician orders.
• Perform routine audits for resident care and medication management.
