Qualifications

• Graduate of a state-approved school of practical nursing or active state certification in medication administration.
• At least 1 year of nursing experience, preferably.
• Experience in geriatric, psychiatric or rehabilitation nursing.
• Certified in CPR and First Aid.
• Willingness to become trained in state assessments and service plans.

Responsibilities

• Maintain liaison with residents, families and departments to plan resident care.
• Ensure services comply with Federal and State policies governing resident care.
• Deliver direct resident care and supervise care associates.
• Report changes in resident condition to the Resident Care Director.
• Assist with staffing plans and completion of departmental schedules.
• Perform routine resident care rounds and document needs.
