Job summary

Meadowbrook Respiratory and Nursing Center is seeking a Social Worker to oversee the development, implementation, supervision and ongoing evaluation of residents' social services. The role ensures services comply with state and federal regulations and are integrated into the comprehensive care plan.

The Social Worker will promote resident dignity, advocate for rights, coordinate discharge planning, and connect residents with community resources.

Qualifications

• Develop and oversee social services plans for residents.
• Coordinate with care teams to integrate psychosocial needs into care planning.
• Identify community resources and assist with transitions to appropriate services.
• Maintain accurate documentation of social work actions in medical records.

Responsibilities

• Oversee development, implementation, supervision and evaluation of residents' social services.
• Identify and provide medically-related social services in compliance with state and federal regulations.
• Complete social services component of comprehensive resident assessments.
• Contribute to and direct social services goals in the care plan.
• Promote non-pharmacological approaches to meet psychosocial needs.
• Encourage resident and family participation in care plan meetings.
• Assist with preadmission services and interdisciplinary evaluations.
• Facilitate discharge planning and connection to community-based services.
• Assist residents in locating financial, legal, and other resources.
• Interpret and report psychosocial needs to the care team and document in records.
• Engage in Advance Care Planning and review Advance Directives regularly.
• Inform staff of code status and end-of-life wishes; educate residents and representatives.
• Assist with policy development and staff training on Resident’s Rights.
