Job summary

The Meadows on University is seeking a qualified Social Service Worker to support planning and coordination of social services for residents in our facility. Qualifications

• Bachelor's Degree in Social Work (BSW) required or a Bachelor’s Degree in a human services field with one year of supervised social work experience in health care.

Responsibilities

• Assist in planning, developing, organizing and directing social services with admin, medical and nursing staff.
• Develop and administer department policies and procedures; review annually.
• Maintain social work progress notes indicating response to treatment plans.
• Coordinate social services with other departments and community resources.
• Assist in discharge planning and arranging outpatient services as ordered.
• Provide information to residents/families about Medicare/Medicaid and financial assistance.
