Job summary

The Meadows on University in Fargo, ND seeks a licensed Social Worker to assist planning, developing, and directing the facility's social services in accordance with federal, state, and local standards.

You will work with medical and nursing staff, coordinate policies, help residents and families with Medicare/Medicaid options, and support discharge planning and community referrals to promote residents' well‑being.

Qualifications

• Ability to read and understand procedure and instruction manuals.
• Basic math skills for calculations related to documentation and reporting.
• Ability to solve practical problems and interpret written/oral instructions.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating social services for residents.
• Meet with administration, medical and nursing staff to plan services.
• Coordinate policies and procedures for the social services department.
• Provide information on Medicare/Medicaid and other financial assistance.
• Assist with discharge planning and community referrals.
• Maintain confidential resident care information and progress notes.
• Support staff and residents with emotional and psychosocial needs.
