Job summary

Meadowbrook Care Center in Texas is seeking a Part-Time Licensed Social Worker to support the emotional and social well-being of our residents. Under the Administrator’s direction, you will plan, organize, and develop programs and services that meet residents’ medically related emotional and social needs in a person‑centered care environment.

Qualifications

• Must be a Licensed Social Worker in good standing.
• Completion of secondary education at an accredited school of Social Work.
• Prior supervisory experience preferred.
• Experience in geriatric, adult, or long‑term care social work is strongly desired.
• Excellent communication and interpersonal skills with residents, families, and team members.

Responsibilities

• Assess and support the emotional and social needs of residents on an individual basis.
• Maintain accurate documentation in resident medical records according to facility policies and federal/state regulations.
• Collaborate with interdisciplinary teams to coordinate continuity of care.
• Serve as a liaison between residents, families, and community agencies.
• Participate in care planning and discharge planning processes.
• Other duties as assigned by the Administrator.
