Job summary

Paramount Care Centers in New Jersey is seeking a Social Worker to provide counseling and support services for residents and families, and to coordinate comprehensive social plans within the facility. Responsibilities include planning discharge, maintaining records, collaborating with the treatment team, and ensuring residents’ rights and safety.

A Bachelor's in Social Work or related field is required; state certifications may be needed.

Qualifications

• Bachelor’s degree in Social Work or a related human services field (Sociology, Psychology, etc.).
• Certifications required by state regulations.
• Knowledge of family assessment and interventions is preferred.

Responsibilities

• Assists with planning and implementing a comprehensive social services program including counseling.
• Identifies residents' social needs and maintains related records.
• Consults with Director of Social Services and other department heads.
• Works with residents and families facing personal or environmental difficulties.
• Promotes residents' physical and mental health and progression of care.
• Maintains written records of consultations and services provided.
• Evaluates social needs and plans care, updating medical records.
• Plans and facilitates family meetings for discharge planning.
• Collects social data at admission and shares with care team.
• Acts as liaison between residents, families, outside agencies, and administration.
• Documents reports; assists with changes in financial status if needed.
• Coordinates discharge planning and organized discharge plans.
• Ensures safety compliance with policies, codes, MSDS knowledge.
• Ensures highest quality of service in a compassionate environment.
• Performs other duties as requested.
