Presbyterian Communities of South Carolina seeks a dedicated Social Services Coordinator to assist residents and families with transitions to new living arrangements. You will serve as first contact for concerns, support communication with families, and help with future planning and signatures on care plans.

You will coordinate discharges, navigate electronic health records, and ensure timely documentation in accordance with State and Federal regulations.

Qualificações

• Coordinate all discharges within the Community.
• Navigate through electronic health records and input required documentation as per the State and Federal regulations.
• Complete and submit required documents and reports on or before established deadlines in compliance with State, Federal and organizational regulations and guidelines.
• Maintain knowledge of applicable Medicare regulations and procedures.

Responsabilidades

• Serve as first point of contact related to concerns of residents, patients, and/or family members; initiate resolutions.
• Encourage family contact and communicate with families on an as needed basis; assist residents and patients with various forms of communication to family.
• Assist residents, patients and their families with future planning.
• Review Bill of Rights with residents, patients and their families; obtain appropriate signatures.
• Review care plan with appropriate party and obtain appropriate signatures.
• Gather social information related to residents and patients.
• Maintain awareness of current Community resources available to residents, patients, and family members.
