Eden East is seeking a Social Services Coordinator to support the social services team in resident care, interviews, and discharge planning. You will document communications and coordinate with the interdisciplinary team to ensure smooth transitions and person-centered care.

Responsibilities include advising residents and families on Medicare/Medicaid and assisting with Medicaid applications, while upholding state requirements and privacy standards. A licensed PA Social Worker is a strong plus.

Qualifikationen

• Bachelor's degree in Social Work or related field.
• Licensed Social Work certificate in PA is a plus.
• 1 year of supervised social work experience in health care preferred.
• Promote cooperation between social services and other departments.
• Professional on the phone and in person when representing the company.

Aufgaben

• Conduct resident interviews to gather social, emotional, and financial needs.
• Assist in discharge planning and coordinate with providers, families, and resources.
• Document interviews, calls, and care conferences daily per policies.
• Coordinate with nurses, therapists, and social workers for care planning.
• Provide information on Medicare, Medicaid, and financial assistance.
