Job summary

The Landings of Rocky Mount Mills is seeking a Resident Care Coordinator to coordinate care services, manage care plans, and ensure regulatory compliance to promote a safe, person-centered environment for residents. You will partner with the Executive Director and care team to deliver high-quality care.

Ideal candidates have 2+ years in senior living or healthcare, an active nursing license, and strong leadership and communication skills.

Qualifications

• Associate's or Bachelor's degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years of experience in a senior living, assisted living, memory care, or healthcare setting.
• Previous supervisory or leadership experience in a healthcare environment is highly desirable.
• Proficiency in electronic health records (EHR) systems and basic computer skills.

Responsibilities

• Care Plan Development & Management: Assess residents' needs and develop ISPs in collaboration with clinical staff, residents, and families.
• Care Plan Development & Management: Monitor and adjust care plans as needs change, ensuring compliance.
• Staff Supervision & Training: Provide leadership, training, and guidance to caregiving staff; ensure adherence to care protocols.
• Staff Supervision & Training: Support staff scheduling to maintain proper staffing levels.
• Resident & Family Engagement: Serve as primary contact for residents and families; facilitate meetings and conferences.
• Regulatory Compliance & Quality Assurance: Ensure compliance with state regulations and company policies; participate in audits.
• Collaboration: Work with nursing, physicians, therapists, and leadership to coordinate care and resident satisfaction.
